Addiction Aftercare Planning

After completing addiction treatment, successful long-term recovery hinges on transitioning back into day-to-day life. Without an aftercare plan in place, people with substance misuse disorders are more likely to suffer a relapse and fall back into unhealthy habits. At Ken Seeley Communities, we provide comprehensive aftercare planning that includes a combination of therapy and education to help clients develop healthy coping and relapse prevention skills.

The Path to Sobriety

Our clinical and discharge teams provide clients with a customized roadmap to help them navigate their recovery journey. To create the path for maintaining their hard-earned sobriety, we connect clients with resources such as the following.

  • Group and individual therapy: Therapy sessions provide a safe and structured environment to address the underlying causes of addiction and work through co-occurring mental health disorders.
  • Sober living arrangements: In the early stages of recovery, structure and accountability are essential. Sober living during outpatient treatment or following completion of residential rehab offers an added layer of support that is so crucial to success.
  • Outpatient treatment: Outpatient treatment adds another layer onto the foundation clients built during residential treatment. Clients can learn life skills and self-esteem that can help them re-acclimatize to the challenges of daily living in the real world.
  • Support groups: Support groups provide support and accountability for recovering addicts. There are many support group models, including both traditional 12-step programs and 12-step alternatives.
